I couldn't figure this out on the forums, regardless of how much I asked..
No one seemed to know where Casey Bones drops.
So here you guys go:
All I know is so far it's confirmed to drop on very hard from Casey Bones, you do not need a golden ticket to get him.

He only drops in Hell mode. We don't have Hell mode yet so no one can get him. Case closed.

Thats not true. I seen people get him in other difficulties. One guy got him on Hard mode. So its possible...just a low drop rate.
